Kailua and Kaneohe together have 36 licensed care homes, 2 assisted living facilities and 3 certified nursing homes. Kaneohe is the windward care hub; Kailua has few licensed settings of its own.
Two towns, very different numbers
Windward families often assume they will have to drive over the Pali or through the tunnels for care. The state's records show more windward options than that, but they are unevenly spread.

Kaneohe is the hub
Kaneohe holds most of the windward side's licensed care: two assisted living facilities, three nursing homes and about two dozen care homes, most of them expanded. One of the assisted living facilities shares its address with a nursing home, which can make a later move to nursing care shorter. For a windward family that wants a parent close by, Kaneohe usually has something at every level of care.
Adventist Health Castle Medical Center in Kailua is the nearest hospital for most Kaneohe addresses, a short drive down Kamehameha Highway or Kaneohe Bay Drive.
Kailua has little of its own
Kailua has around a dozen licensed care homes, almost no foster family homes and no licensed assisted living or nursing home inside the town. That reflects housing prices and the town's mix of homes more than demand. Kailua families usually compare a handful of local care homes with the Kaneohe options, which are 10 to 15 minutes away.
For a parent who wants to stay in Kailua specifically, start calling the local care homes early; with so few, openings are irregular.
Foster family homes are scarce on the windward side
Of Oahu's 1,111 certified foster family homes, only a few are in Kailua or Kaneohe. For a Med-QUEST member at nursing level who wants to stay windward, an expanded ARCH or a nursing home is the more realistic option. A case management agency can tell you whether any windward foster family home has an opening.
If the family is flexible, foster family homes are plentiful in Leeward towns, but think carefully about visiting distance before choosing one across the island.
Windward hospitals and specialists
Adventist Health Castle Medical Center in Kailua handles most windward emergencies and admissions. Many specialists, cancer care and some surgeries are in town at Queen's, Straub, Kuakini or Kaiser Moanalua, so a windward care home needs a reliable plan for getting residents over the Pali or through the H-3 tunnels. Ask each home who drives, how often, and whether a family member is expected to come along.
When to look beyond the windward side
If a windward parent needs a nursing-level home placement and no expanded home or foster family home has an opening nearby, the next nearest options are often in Kalihi, Salt Lake or Aiea, about 20 to 30 minutes away outside rush hour. Consider a temporary placement there with a plan to move back when a windward opening comes up, especially if the family's main visitors live on the windward side.
Questions for a windward placement
Ask each windward home how residents get to appointments in town, whether it has a plan for heavy rain and road closures, and who the substitute care giver is. Ask whether the home works with Castle's discharge planners, and how quickly a case manager can visit for an expanded resident. For assisted living in Kaneohe, ask whether the building's nursing home neighbor gives residents priority for beds. Read the latest OHCA inspection report or CMS record for every finalist before visiting.
Windward families also tend to have strong ties to particular communities, from Waimanalo and Kahaluu to the Marine base at Kaneohe Bay. If your parent's church, friends or regular activities are on the windward side, count that as part of their care. A slightly less convenient home that keeps those ties may serve them better than a closer one that breaks them. Ask whether the home can bring residents to services or community events, and whether friends are welcome to visit at any reasonable hour; in a small household, an open door for visitors is part of good care.
What to ask windward homes
Ask about access as well as care. Some windward homes are on hillsides or narrow streets; ask how residents in wheelchairs get in and out, and check OHCA's wheelchair column for each home. Ask how the home handles appointments in town, since many specialists are in Honolulu. And ask what happens when the Pali or Likelike is closed.
