TL;DR

Aid and Attendance raises VA's maximum pension rate to $29,093 a year for a veteran with no dependents (2026). VA subtracts unreimbursed care costs from income, so a veteran whose care home fees exceed their income can receive close to the maximum, about $2,424 a month.

The rule behind the math

VA pension with Aid and Attendance is not a flat benefit. VA sets a maximum annual pension rate (MAPR), then pays the difference between that rate and the claimant's countable income. Countable income is income minus unreimbursed medical expenses, and for someone in a care home, the care home fees count as medical expenses when the care is needed because of a disability.

2026 rate (Dec 1, 2025 to Nov 30, 2026)AnnualMonthly
Veteran with no dependents, Aid and Attendance$29,093$2,424
Veteran with one dependent, Aid and Attendance$34,488$2,874
Two veterans married to each other, both A&A$46,143$3,845
Surviving spouse, no dependents, Aid and Attendance$18,697$1,558
Veteran with no dependents, Housebound$21,313$1,776

A worked example

Take a hypothetical Oahu veteran, single, with no dependents. He receives $2,100 a month from Social Security and a small pension, $25,200 a year. He moves into a care home charging a monthly rate the family has in writing; for the example, assume the care costs he pays come to more than his income.

Step one: annual income, $25,200. Step two: subtract unreimbursed medical expenses, here the care home fees plus Medicare premiums. Because those exceed his income, countable income falls to zero. Step three: VA pays the MAPR minus countable income, $29,093 minus $0, or about $2,424 a month. If his care costs were lower, countable income would be higher and the payment smaller.

VA applies a deductible to medical expenses and has detailed rules about what counts, so the real calculation will differ. The example shows the shape, not his exact award.

The other tests

The veteran must have qualifying wartime-era service and discharge, and meet the need test: needing help with daily activities, being bedridden, being in a nursing home because of incapacity, or having limited eyesight. The 2026 net worth limit is $163,699, counting assets plus annual income. VA also reviews asset transfers in the three years before the claim.

Oahu specifics

Oahu has a large veteran population, many living near Pearl Harbor, Kaneohe, Schofield and Hickam. Care home fees on the island are high enough that many veterans who need daily help will have countable income near zero, which is exactly the situation Aid and Attendance is designed for. For nursing care, the Daniel K. Akaka State Veterans Home in Kapolei is a CMS-certified option for eligible veterans; it is one of the 27 certified nursing homes in our directory.

A surviving spouse example

Consider a hypothetical surviving spouse of a Korea-era veteran living in a Pearl City care home. Her income is $1,600 a month from Social Security, $19,200 a year. Her care home fees and Medicare premiums exceed that amount, so her countable income falls to zero after the medical expense deduction. The 2026 maximum for a surviving spouse with no dependents and Aid and Attendance is $18,697 a year, so she could receive close to $1,558 a month, which together with her Social Security goes a long way toward the care home rate.

As with the veteran example, VA's deductible and documentation rules will change the exact figure. The point is that care costs, not just income, decide the award.

Documents VA will want

VA will want proof of service (the DD-214), marriage and death certificates for surviving spouse claims, income and asset statements, the care home agreement showing the monthly rate, receipts, and a medical statement of the need for care. An accredited service officer can help assemble these at no charge.

Common misunderstandings

Families often think Aid and Attendance is only for nursing homes, that it is a fixed monthly payment, or that a veteran must have been injured in service. None is true. Pension is needs-based and requires wartime-era service, not a service-connected disability; it can help pay for a care home, assisted living or home care; and the amount depends on income and care costs. Veterans with a service-connected disability may instead qualify for VA compensation, which follows different rules.

Timing

VA decisions can take months. Filing an intent to file can protect the start date while documents are gathered. Bring the DD-214, income and asset records, the care home agreement and receipts. Many families pay the care home from savings while the claim is pending and receive a retroactive payment later.
