Hawaii's 2025 assisted living median of $145,065 a year (about $12,089 a month) ranks first nationally, roughly twice the U.S. median. The number reflects a small group of large Honolulu buildings. Most Oahu families who need daily help use care homes instead, which no survey prices.
The figures
CareScout's 2025 Cost of Care Survey, released in March 2026, ranks Hawaii first among states for assisted living cost, fourth for nursing homes and ninth for home care.
| Care type (Hawaii, 2025) | Annual median | U.S. median | Hawaii rank |
|---|---|---|---|
| Assisted living | $145,065 | $74,400 | 1 |
| Nursing home, semi-private | $185,679 | $114,975 | 4 |
| Nursing home, private | $196,735 | $129,575 | 4 |
| Home care (44 hrs/week) | $93,808 | $80,080 | 9 |
| Adult day health care | $30,615 | $24,700 | 15 |
Assisted living rose 7% from 2024, the fastest increase of any Hawaii category. Home care fell 2%.
A small, top-heavy market
Part of the explanation is how few licensed assisted living facilities Hawaii has. OHCA's April 2026 roster lists 14 on Oahu, most of them large buildings in Honolulu, several on Kalakaua Avenue and in Makiki, and some part of continuing-care campuses with nursing licenses alongside. A median drawn from a small group of mostly full-service, often high-rise buildings will be high.
Hawaii's rules also set a high floor: every licensed assisted living unit must be a private apartment of at least 220 square feet plus a bathroom, with a kitchenette and a 24-hour call system, and the building must have direct-care staff on site at all hours. That is expensive to build and staff anywhere, and more so in Honolulu.
The cost of land and labor
Construction and land costs in Honolulu are among the highest in the country, and care work competes for staff in a tight labor market with a high cost of living. Every dollar of those costs ends up in the monthly rate. Mainland chains that dominate assisted living elsewhere have largely not built on Oahu, so there is less price competition.
Where most families actually go
The number that matters for most Oahu families is not the assisted living median but the price of a care home, and no survey publishes that. With 455 licensed care homes and 1,111 foster family homes on the island, small homes care for most people who need daily help. Care homes usually quote one monthly rate covering room, meals, supervision and personal care, and families who compare quotes generally find it below the assisted living median for a similar level of help.
Assisted living still makes sense for some families: a couple who want to share a private apartment, a parent who values independence and activities, or someone with long-term care insurance that covers it. For everyone else, getting three care home quotes is the most useful price research you can do.
Couples and assisted living
One group for whom assisted living often makes sense despite the price is couples. In a care home, spouses may be able to share a room, but a private apartment with a kitchenette is rare outside assisted living. Most buildings charge a second-person fee rather than two full rates, which narrows the gap. If one spouse needs much more care than the other, ask how the building prices care for each person separately.
Insurance and benefits that help
Long-term care insurance often pays toward licensed assisted living once the policy's benefit trigger is met, usually help with two activities of daily living or cognitive impairment, after an elimination period. VA pension with Aid and Attendance can help eligible veterans and surviving spouses because care costs reduce countable income. Ask each building whether it accepts Med-QUEST members for assisted living services through a QUEST Integration plan; if not, plan for what happens when savings run low.
How to compare a quote with the median
When a building quotes a price, compare like with like. The CareScout median reflects a private one-bedroom or studio unit with a typical level of care. A quote for a larger apartment, a second person or a high care level will be above it; a base rent without care fees will look below it. Ask for the all-in monthly figure for your parent's actual needs, and ask for the last three years of rate increases.
What to ask about price
Whichever setting you look at, ask for the rate in writing with your parent's actual care needs included, every extra charge, and the notice period for increases. In assisted living, ask how care is priced on top of rent. In a care home, ask what triggers a higher rate. In both, ask what happens if savings run out.
